same thing happened to a bunch of us. they fixed the issue without needing to send anything back. just contact them

But when I look into my order history my first two orders are for the first single - one with the box and one alone... Or is this what happened to everyone else?

I've already ordered no.2 again...

My orders were correct, they made the error. I think you should still contact them even if you think you made the error. They were very pleasant about it, considering they had to ship a new single to America and not get the extra one back (which is still available for the shipping to a fellow fan).

Having purchased the I Tunes download of Big Sister, without accompanying artwork or text, I'm interested to know who plays piano on this recording - is it John Trier or someone else :?: . Grateful if anyone can enlighten me.

The 10" singles don't have artwork or detail of who does what, they are just the same as the old 7" vinyl singles, writing and publication details on the record label. The sleeve is basically the Parlophone standard with the Richard Hawley Singles Club logo on it but no other info.
